Yes, the Metro version of IE doesn't support add-ons, so this is expected.

You have turned on blocking from the add-on's settings at some point. Go to the settings page and switch it back to normal warnings instead.

Yes, you can solve the problem by removing the add-on from Chrome and reinstalling it.
Are you using the dev release of Chrome by any chance? There was apparently something not quite right with the Web Store and the add-on update failed for some people.

I assume the online tracking rating category replaced the oldspyware / adware comment category.
No, I believe that was combined to the malware category. You can read the description by hovering over the category in the rating window.
