Elements below the rating sliders tend to shift position 1-2x, apparently on mouse-over, then stabilize.
The Agree / Disagree icons are way too transparent.
I see no other problems.

This might be useful on a pro rata basis if the comment categories were significantly refined; however this runs the risk that complexity could degrade the impact of comments.
