How do I make sure I NEVER see a site again, even if WOT rates it trustworthY?

I ended up on a porn site that I NEVER NEVER want to view again, nor do I want anyone in my family.

How do I make sure that it doesn't happen again? The WOT warning page didn't popup.

The add-on doesn't warn you about child safety by default. If you prefer to warned about poor child safety, you can configure the warning behavior to your liking on the Warnings page of the add-on settings.

If you also select the "Protect me if no rating is available" option, the add-on will warn you of sites that haven't been rated. Finally, if you come across a website whose child safety rating is wrong, you can just rate the site yourself and the add-on warns you if you ever happen to visit it again.

Of course, if you want to be sure that nothing is shown, you'll have to set the add-on to block access to sites instead of just showing the transparent warning layer.

You can block a site several ways. The simplest is that you go to your WOT preferences, and set it to block sites with low child safety.
There are other ways to do it than WOT. For example, you can configure your firewall to block outgoing connections to that site. You may also use OpenDNS (www.opendns.com) as your DNS server, which allows you to block sites, or you can even block all porn sites.

You can simply rate the site as red. I have done this on sites rated green, but because I rated it red, it blocks me by bringing up the warning screen. Simple.

WOAH there Nellie ..... you can't rate a site as red, just because you don't want to visit it again. That kinda defeats the object of the damn rating system in the first place.

You should ONLY rate sites red/green/whatever, based on the evidence pertaining to the site itself, and then in relation to the rating you are giving it. Otherwise, whats the point?
